I hereby express my desire to affiliate with the Universal Grand Lodge.

If my request for affiliation is accepted, I make a formal commitment to consider the teachings sent to me as confidential. In the event that, for any reason whatsoever, my affiliation is terminated, I also commit to returning to the Order's headquarters the monographs, manifestos, monthly bulletins and, in general, all documents that may have been sent to me during the time my affiliation lasted.

If I am not admitted into the Order, I will be notified confidentially, and the entry fee as well as the membership fee I paid will be refunded to me.
